Your Annual Health Review
Annual reviews help us check how you are managing your condition, review your treatment, and make sure you are getting the care and support you need.
When will I be invited?
We invite patients for annual reviews throughout the year.
Some patients may be invited sooner than others based on their health needs or the results of previous reviews.
If you are due a review, we will contact you with information about what you need to do next.
Please make sure we have your current contact details so that you receive any invitations and reminders.
What happens during an annual review?
The type of review you need will depend on your health condition.
Some reviews include:
- A blood test
- A blood pressure check
- A urine sample
- A questionnaire
- A review with a nurse or other healthcare professional
We will tell you what you need to do when we invite you for your review.
Booking your review
Many review appointments can be booked online using the NHS App or Patient Access.
If you are unsure which appointment to book, please contact us online or call the surgery.
