Annual Review Appointment Information
Supporting patients to make the right choices to manage their health
We believe in supporting patients to make the right choices to manage their health. To achieve this, we offer patients with high blood pressure, heart problems, stroke/TIA, diabetes, dementia, asthma and COPD an annual review appointment, providing us the opportunity to review your condition.
Reviews differ, some consist of a blood test, blood pressure check, urine dip and, when required, will be followed up with a telephone call with a practice nurse. Other reviews require your input to complete a questionnaire (Asthma, COPD, Heart Failure) and a nurse will review the results and if required will call you to discuss any issues.
In general, we aim to invite you in for a review, or, to send out questionnaires, during the month of your birth. This process allows us to manage patient flow through the surgery and ensures that we see you regularly each year.
We ask, if possible, that you book your Annual Review appointments online, (during your birthday month) via NHS App or Patient Access as this helps to protect the number of incoming calls to the surgery.
Please see the table below for the type of appointment you should book according to your chronic disease.
Are you aware of the national service called Pharmacy First? You can visit your pharmacy for a range of minor ailments such as Sore throat (5yrs and over), Sinusitis (12yrs and over), uncomplicated urinary tract infections (women 16-64yrs) ear infections (1-17yrs). The pharmacist can often recommend treatment, or when this is not possible will refer your problem back to your GP.
You can also use your local pharmacy for blood pressure checks (both 24hr and one-off BP monitoring) - the readings are fed back onto your medical record. Paignton library is currently offering a free blood pressure machine loan service if you are asked or would like to monitor your blood pressure at home.
Our prescription enquiry line is open from 9.30am-12.30pm, Monday to Friday. Please use the NHS App, Patient Access or Klinik (our online form) to order prescriptions.
Text messaging has seen various innovations that have expanded its capabilities beyond simple text messaging and, as a result, has become an invaluable tool used in the Practice.
We kindly ask that your mobile number on your medical record is up to date. If you do not consent to receiving texts from us, please let us know, and note, that declining texts may prevent you from receiving appointment reminders as well as other future text correspondence.
Quitting smoking is one of the best things you can do for your health, no matter your age or how long you have smoked. If you are a smoker and would like support to quit you can visit
